How to Fill Out Checks to the IRS and State Government

Writing a check to pay your taxes is something you want to get right, but don’t worry, it's a straightforward process. Follow these easy steps to ensure your tax payment check is correctly completed and reaches the IRS without any issues.

First, procedures are subject to change, so we always recommend checking for any updates on the official IRS website and with respective state government’s Department of Revenue.

For IRS payments, we’ve included an addressed envelope in your tax folder for your convenience. If you prefer to pay online, you can do so here: IRS Online Payment

  • Make your check, money order or cashier's check payable to United States Treasury.

    • Please note: Do not send cash through the mail.

  • Enter the amount using all numbers ($###.##).

  • Make sure your check or money order includes the following information:

    • Your name and address

    • Daytime phone number

    • Social Security number (the SSN shown first if it's a joint return) or employer identification number

    • Tax year

    • Related tax form or notice number

  • Make sure you sign and date. This can be easily overlooked as you’re writing more information on this check than usual.

Oregon Department of Revenue

We’ve included an addressed envelope in your tax folder for your convenience. If you prefer to pay online, you can do so here: Oregon DOR Online Payment.

  • Make check payable to: Oregon Department of Revenue

    • Please note: Do not send cash through the mail.

  • Write the form name, e.g. “Form OR-40-V,”

  • Your daytime phone,

  • The last four digits of your SSN or ITIN,

  • And the tax year on your payment.
